Maximum technology

The pen does not make the plan.

DHI — Direct Hair Implantation — is an implantation approach that uses a specialised implanter pen to place prepared follicular units into the recipient area. It is a useful technique. It is not a shortcut to a natural result.

The outcome still depends on the donor plan, the hairline design, the placement decisions and the people carrying them out. At Maximum Istanbul, technology follows the treatment plan — never the other way around.

  • 16 years of senior hair-restoration experience.
  • A maximum of two patients per day.
  • One complete treatment standard.

02 The method

What DHI actually means.

In DHI, follicular units are first extracted from the donor area, typically using an FUE extraction method. Those grafts are then prepared and loaded into an implanter device for placement in the recipient area.

Depending on the device and clinical technique, the pen may create the implantation opening as the graft is placed. This differs from approaches in which recipient sites are made separately before graft insertion.

DHI describes an implantation method. It does not mean the follicles are extracted without FUE, that surgery is scar-free, or that a particular result is guaranteed.

03 Maximum precision

The tool is precise. The decisions are human.

An implanter pen gives the operator a way to position grafts with controlled handling. But the pen cannot choose the right hairline, select donor grafts or judge the distribution that will look natural years later.

Angle, direction, depth, spacing and graft choice still require careful decisions. The aim is for transplanted hair to follow a believable pattern and work with the surrounding hair.

At Maximum, every instrument is selected for a reason. Precision is a planning discipline, not a product label.

04 Existing hair

Work with the hair that is already there.

Many patients want to strengthen a thinning hairline or mid-scalp rather than fill a completely bald area. In these cases, the treatment plan must account for native hairs that are still growing — including finer, miniaturised hairs.

DHI implantation may be considered in selected areas where working among existing hairs is relevant. It is not automatically the safest or best method for every patient, and implantation still carries a risk of temporary shock loss or injury to nearby follicles.

The right question is not simply, ‘Can DHI be used here?’ It is, ‘How should this area be treated without losing sight of the existing hair and long-term plan?’

05 Clear terminology

DHI and FUE are not opposites.

FUE refers to how individual follicular units are removed from the donor area. DHI refers to one way those units can be implanted. They are not mutually exclusive procedures.

Likewise, ‘Sapphire FUE’ usually describes the use of sapphire-tipped blades for recipient-site creation, not a different way to extract follicles. Comparing ‘FUE versus DHI’ as if one replaces the other can therefore be misleading.

A well-designed treatment may use FUE extraction and different recipient-site or implantation approaches in different zones.

06 The Maximum Standard

Two techniques. One treatment strategy.

For suitable scalp patients, our standard planning combines Sapphire FUE and DHI according to the needs of the individual case. Some areas may benefit from pre-made recipient sites; others may be approached with an implanter pen.

We do not choose a technique because it sounds more expensive. We choose the approach based on the area, existing hair, graft characteristics and clinical assessment.

DHI is not sold as a premium technique upgrade. The method is part of the treatment planning where appropriate.

07 Natural design

The hairline is more than a row of grafts.

A natural hairline depends on shape, irregularity, softness and how the hairs emerge from the scalp. No implantation device can decide those details on its own.

The first transition zone often calls for careful graft selection and restrained placement. Behind it, the plan must balance visible coverage with the donor supply available for future needs.

We assess the face, current hair, likely progression of loss and donor characteristics before deciding how a hairline should be restored.

08 Donor management

A donor area you still need tomorrow.

Every graft placed in the recipient area comes from a finite donor supply. A good transplant plan has to consider both sides of that equation.

DHI does not increase the number of grafts your donor area can safely provide. Nor does it remove the need for careful extraction, graft handling or a realistic assessment of future hair loss.

The objective is thoughtful distribution — not the biggest possible graft number on a sales message.

09 Maximum expertise

Experience leads. Technology supports.

Our senior hair-restoration expert brings 16 years of experience to consultation and treatment planning, including hairline design and the key planning decisions that shape each case.

The procedure is carried out with the involvement and oversight of appropriately licensed medical professionals. Each clinical task is assigned according to qualifications, local rules and the treating provider’s approved protocol.

What matters is not a pen in a photograph. It is the judgment, coordination and care behind every step.

10 Maximum attention

Two patients a day. Never an assembly line.

We limit our schedule to a maximum of two patients per day. This is a deliberate part of how we organise consultations, preparation and patient attention.

A DHI procedure involves many small decisions and careful coordination between the people handling, loading and implanting grafts. The team and the pace matter.

You should know who is responsible for your treatment, how the day is structured and who to contact afterwards.

11 The patient journey

Your procedure, step by step.

  1. 01

    Assessment

We review your photos, medical background, hair-loss pattern, donor area and goals. Suitability and the final plan are confirmed by the treating medical team.

  2. 02

    Design and preparation

The recipient plan and donor strategy are agreed, with the anaesthesia approach determined by the medical team.

  3. 03

    Extraction and graft preparation

Individual follicular units are extracted and handled for implantation according to the clinical protocol.

  4. 04

    Placement

DHI pens may be used in selected recipient zones, with other implantation approaches used where appropriate.

  5. 05

    Review and aftercare

The team checks the treated areas, explains washing and medication instructions and remains available for follow-up.

12 Healing & growth

Recovery is biology, not a brand name.

Redness, swelling, tenderness and small crusts can occur after a hair transplant, including when DHI is used. Healing and discomfort vary by patient and treatment extent.

Some transplanted hairs may shed in the weeks after treatment. Visible new growth often begins over the following months, while density and maturity continue to change over approximately 12–18 months.

Neither an implanter pen nor a sapphire blade can promise a scar-free procedure, zero shock loss or a specific healing speed. Your treating team will provide instructions matched to your case.

13 Clear pricing

€2,500. No DHI upgrade.

Our complete scalp hair-transplant treatment is priced at €2,500. For suitable patients, combined Sapphire FUE + DHI planning is part of the standard — not an add-on sold after you arrive.

The planned international patient experience includes the treatment, PRP and Graft-Tech stem-cell component where medically appropriate, two hotel nights, scheduled airport transfers and aftercare support, subject to the confirmed written quotation.

Any optional anaesthesia services, exclusions, individual medical requirements and booking terms must be made clear before payment. The final clinical plan is determined after assessment.

14 FAQ

Frequently asked questions.

Is DHI better than FUE?

Not as a universal rule. FUE usually describes extraction, while DHI describes an implantation approach. A meaningful comparison depends on the patient, recipient area, graft plan and team experience.

Does DHI use a special pen?

Yes. DHI implantation uses an implanter device that holds a prepared follicular unit and allows it to be placed into the recipient area. The exact model and method vary by treating team.

Is DHI suitable for thinning hair?

It may be considered for selected areas with existing hair, but it is not automatically the best choice. The medical team must assess the native follicles, donor supply and risk of shock loss.

Do I need to shave my head for DHI?

Not always. Shaving requirements depend on the donor area, recipient area, graft number and clinical approach. A no-shave or partial-shave option is not guaranteed.

Does DHI leave scars?

FUE extraction can leave small donor-site scars, and recipient areas need time to heal. No hair transplant method should be described as completely scar-free.

Is DHI less painful?

The procedure is typically performed with local anaesthesia. Sensations during injections and recovery vary; DHI alone does not guarantee a painless experience.

Does DHI give higher density?

No technique guarantees a particular density. Safe density depends on donor capacity, recipient blood supply, existing hair and careful placement.

Can DHI and Sapphire FUE be combined?

Yes. FUE extraction can be used alongside different recipient-site and implantation methods. Maximum plans combined Sapphire FUE + DHI for suitable patients, according to the area being treated.

How long does it take to see results?

Initial shedding is common. New growth usually develops over the following months, with changes continuing for roughly 12–18 months. Individual timelines vary.

How much does DHI cost at Maximum Istanbul?

The advertised complete scalp treatment rate is €2,500, with combined-technique planning included for suitable patients. Confirm your written quotation and any optional services before booking.

Who performs the DHI implantation?

The licensed treating provider assigns the clinical tasks to appropriately qualified personnel under its medical protocol. Ask for the specific team roles during your consultation.

Can DHI treat the crown?

It can be considered as part of a crown restoration plan, but the crown’s growth pattern and graft requirements need careful assessment. Not every crown can be restored to the density a patient hopes for.

15 Suitability

Is DHI right for you?

There is no single technique that is ideal for everyone. Your hair-loss pattern, native-hair density, donor capacity, medical history and expectations all matter.

A patient with diffuse thinning may need a different plan from someone restoring a bare hairline or a large crown. In some cases, stabilising ongoing hair loss or delaying surgery may be the more sensible recommendation.

Send clear photos and tell us what you want to improve. We can explain the options and arrange a proper clinical suitability review.
